Psychogeriatric care
Specialist hospital care for people with high needs and, often, challenging behaviours.
What it is
Specialist (psychogeriatric) hospital care is the most specialised of the four levels of full-time residential care. Health New Zealand describes it as care for people with high needs and, often, challenging or noisy behaviours, with a minimum of two staff on duty at all times: one registered nurse and one caregiver.
Places are scarce. Only 36 care homes on this site offer it, in 15 of the 18 regions.
Who it suits
Someone whose needs assessment recommends specialist hospital care. The assessment decides this level, not the family or the home.
What it costs
Our needs assessment guide notes that the weekly maximum a home can charge for contracted care is the same at every level of care. From 1 July 2026 the cap on a standard room runs from $1,513.33 to $1,634.43 a week across district and city council areas. Ask the home to confirm the rate that applies, and to put any extra charges in writing.
The Residential Care Subsidy (government help with the cost of long-term care) is means-tested by Work and Income. Who qualifies for the subsidy.
How to get it
Start with a needs assessment from your local needs assessment service (NASC, short for Needs Assessment and Service Coordination). You can contact NASC yourself, or ask your GP or the hospital team how referrals work in your area. The assessment works out which level of care is needed; you do not choose the level yourself. Then choose a home with a vacancy at that level. If your first choice is full and the need is urgent, you may have to take a place elsewhere, and you can move to your preferred home later when it has a suitable vacancy. With so few homes, that is more likely here, so join the waiting list of the homes you would accept. When you move in, you sign an Admission Agreement, the contract that sets out your rights and responsibilities.

- Is a standard room available now? If not, will I be charged premium fees while I wait?
- Can you meet my care needs now, and as they change?
- How many staff, and how many registered nurses, are on duty during the day and at night?
- What else will appear on my bill, and is every charge written into the Admission Agreement?
